The prices listed below are effective as of August 1, 2007 and are subject to change without notice.
These goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy items that you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods & services you selected.

Minimum Services of Funeral Director & Staff:         $995.00
This fee is for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This is already included in our charges for direct cremations, immediate burials, & forwarding and receiving remains) This fee includes the following:

A. Personnel available 24 hours a day, 365 days a year to respond to initial call.
B. Arrangement conference.
C. Coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition.
D. Clerical assistance in the completion of various forms & permits associated with a funral.
E. Overhead expenses including, but not limited to, facility maintenance, equipment and inventory costs, insurance & administration expenses & governmental compliance.

Preparation of Remains:
Except in certain cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

2. Dressing, casketing and/or cosmetics of deceased.               $185.00
3. Bathing & handling of un-embalmed remains.                       $125.00

5. Special care for autopsy cases.                                            $245.00

Use of Facilities & Staff:
Use of facilities & staff for visitation (per day). Our services include set-up of visitation area, placement of flowers for display and staff for supervision of visitation.   $255.00.

(We do not charge visitation fees when families have visitation prior to funeral service or same day as the funeral service.)

Use of Facilities & Staff:

Use of facilities and staff for funeral service in our funeral home chapel.   $250.00

Staff and equipment for funeral service to be held in church or place of worship or other place other than our funeral home.   $250.00

Staff and equipment for funeral service to be held at the graveside only.   $205.00

Use of our facilities for memorial service.   $250.00

Transportation:
1. Transfer remains from place of death to the funeral home within 35 mile radius. (Additional mileage is charged a rate of $3.00 per loaded mile.)                                                           $175.00
2. Funeral vehicle, Hearse for funeral service.                           $175.00
3. Family transportation via limo.                                              $175.00
4. Flower van to transport floral arrangements.                         $175.00
5. Service car, Lead car for funeral service.                              $125.00
6. Transfer remains to/from airport.                                          $195.00
7. Transfer remains to crematory.                                             $175.00

Forwarding remains to another funeral home:

This fee includes removal of remains (1/2 of the minimum charge of the funeral director & staff charge), embalming and other preparation of remains a transportation to the Birmingham airport.             $1,970.00

Receiving remains from another funeral home:

This includes 1/2 of the minimum charge of the funeral director and staff charge, transportation from the Birmingham airport, transportation to local cemetry. This fee does not include visitation or funeral services either at the funeral home or place of worship.             $1,225.00

Direct Cremations:
If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard of composition materials. The containers we provide for our client families are made of composition materials (cardboard). Families have the option of cremation caskets which are designed especially for crematiion. Please ask our director for more information if you are interested in this type of service and merchandise.

Direct Cremations:
Our charge for direct cremation with container provided by our funeral home. (All containers must meet the requirements of the crematory. This charge includes:

1. Removal of remains from place of death.
2. Bathing/handling of remains until time for transportation to crematory.
3. 1/2 the minimum charge of funeral director and staff.
4. Transportation to Crematory
5. Cremation fee ($450.00)                                $1,425.00

Direct Cremation with a memorial service at our chapel:       $1675.00

Immediate burials
Our charge for immediate burial without any services is as follows:

1. Immediate burial with minimum casket from our funeral home:   $2,340.00
2. Immediate burial with casket/container furnished by consumer. : $1,345.00
(All caskets/containers provided by consumer must meet state and cemetery requirements.

Alternative Containers:

Caskets:

Outer Burial Containers:
A complete list will be provided at the funeral home, but range in price from $995.00 - $7495.00.

Other merchandise and/or services:
1. Memorial Register Book purchased seperately.             $55.00.

Cash Advances;
Cash Advances are those items that our funeral home pays for you on your behalf in advance. We make no charge for this serice. Most common cash advances are listed below but are not limited to:

1. Opening/closing grave
2. Death Certificates ($12.00 for the first and $4.00 each for additional ones.
3. Obituary Notices
4. Honorariums (clergy, musician, etc.)
5. Any and all items you ask us to include that are not related to our funeral home charges.

Payment Policy of Folsom Funeral Home & Piedmont Memory Gardens

We are a small business and are dependent upon our client families to pay for services and merchandise when rendered. We ask all families to make payment arrangements prior to funeral services. Listed below are the most common forms of payment:

1. Cash/check
2. Credit Cards
3. Pre-Need Funeral arrangements
4. *Insurance Assignments.
5. DISCOUNTS- Discounts will be given in the amount of 3% of the total funeral home charges which does not include cash advances, if payment in full is made by the day of service.

*Verified insurance policies are always accepted as form of payment; however, due to the delayed processing of claims by insurance companies, we must charge our clients 4.5% of the total funeral cost. This cost will be added to your total funeral account.
